The Forward Trust in Cardiff is seeking a Senior Health and Wellbeing Practitioner to lead the Integrated Substance Misuse Team at HMP Usk and HMP Prescoed. You will supervise practitioners, manage a varied caseload and drive recovery outcomes within a prison setting.

Senior Health and Wellbeing Practitioner: HMP Usk and HMP Prescoed

Location: Cardiff

Salary: A GBP 28,742.18 per annum

Vacancy Type: Permanent, Full time

About The Role

Are you passionate about helping people make lasting change? We’re looking for a Senior Health and Wellbeing Practitioner to join our Integrated Substance Misuse Team at HMP Usk and HMP Prescoed, working full time (37.5 hours per week). This is a rewarding leadership role supporting adult males within a prison setting, combining frontline clinical work with team management and service development. You’ll play a key role in supporting individuals with substance misuse and low:level mental health or emotional wellbeing needs, helping them progress through structured recovery journeys and achieve positive outcomes.

This is a chance to step into a senior position where you’ll directly influence recovery journeys while developing and leading a team. You’ll be part of a collaborative, multidisciplinary service focused on meaningful change and positive outcomes.

All prison:based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks. Please note if you have lived overseas within the last 5 years then checks may take longer.
