Job summary

Forward Trust invites applications for a Health and Wellbeing Practitioner based in Cardiff. The role focuses on supporting male adults with substance misuse and low-level mental health or emotional wellbeing needs, delivering both individual and group interventions.

You will manage a caseload, complete assessments, develop care plans and work with partner agencies to enable sustained positive change on recovery journeys.

Qualifications

• Experience in substance misuse, health and wellbeing, or related support setting.
• Relevant knowledge or training in substance misuse and/or mental health/wellbeing.
• Passionate about helping people make positive, lasting changes.
• Excellent communication and interpersonal skills.
• Ability to assess needs, develop care plans and referrals.
• Able to manage a caseload and prioritise competing demands.

Responsibilities

• Manage a caseload of service users and provide ongoing support throughout their journey.
• Work with male adults experiencing substance misuse and low-level mental health or emotional wellbeing needs.
• Provide one-to-one, face-to-face support.
• Deliver and facilitate group interventions.
• Complete assessments and develop care plans with service users.
• Provide advice and guidance around substance misuse, health and wellbeing.
• Identify needs and make referrals to specialist services.
• Collaborate with service users to identify goals and support positive change.
• Monitor progress and contribute to measuring outcomes.
• Work with colleagues and external agencies for coordinated support.
• Attend multi-agency meetings representing Forward Trust.
