Job summary

The Forward Trust is seeking a Health and Wellbeing Practitioner to join the team at HMP Bronzefield in Sutton. You will provide recovery‑oriented health and wellbeing services to adults, addressing low‑level mental health needs and substance misuse within a multidisciplinary setting.

Responsibilities include managing a caseload, delivering one‑to‑one and group sessions, developing SMART care plans, and supporting release planning and continuity of care through partnerships with community

Qualifications

• Experience in mental health or substance misuse services.
• Ability to design and implement SMART care plans.
• Experience delivering structured one-to-one and group interventions.
• Strong IT skills and maintaining accurate electronic records.

Responsibilities

• Managing a caseload and providing consistent, person-centred support.
• Providing harm reduction information and guidance.
• Completing comprehensive assessments and developing SMART care plans.
• Delivering structured one-to-one interventions.
