The Forward Trust in Swansea is seeking a compassionate Recovery Practitioner to support prisoners aged 18 and over within an integrated healthcare framework. You will manage your own caseload, deliver assessments, SMART care plans and evidence-based interventions designed to help clients move forward with their recovery journey.

You will collaborate with healthcare, probation and partner agencies, provide group sessions and maintain accurate records, contributing to service targets and outcomes

Qualifications

• Experience in mental health or substance misuse services.
• Ability to build trusting relationships with clients.
• Understanding of recovery-focused practice and 12-step concepts is a plus.

Responsabilités

• Carry out comprehensive assessments and develop SMART care plans.
• Deliver structured one-to-one interventions and therapeutic groups.
• Provide counselling and information on harm reduction and recovery.
• Work with healthcare, probation and partner agencies for coordinated care.
• Maintain accurate records and contribute to service targets.
