Location: Wallsend Health Centre, North Tyneside
Contract: Fixed-term, 12 months (Secondment)
Hours: 15 hours/week (flexible, 2 days across Monday-Friday)
Salary: £46,148 - £52,809 pro rata per annum (NHS Band 7)

To provide a nurse liaison service within a large multi-disciplinary Community Learning Disability Team (CLDT) in North Tyneside. The role involves embedding physical health and wellbeing needs across disciplines, supporting community services, and promoting health equity for individuals with learning disabilities.
