Job summary

Oakfield Street Surgery in Wales is hiring a pharmacy technician for a 12-month fixed-term post. The role is 25 hours per week and focuses on medicines optimisation, prescription management and medication reviews, under supervision of a Clinical Pharmacist or GP.

Registration with the General Pharmaceutical Council is required. You will liaise with GPs, nurses, community pharmacists and care homes and support audits and QA contracts as part of a multidisciplinary primary care team.

Qualifications

• BTEC/NVQ Level 3 in Pharmaceutical Sciences or equivalent pharmacy technician qualification.
• Registered Pharmacy Technician with the General Pharmaceutical Council.

Responsibilities

• Carry out structured medication monitoring and follow-up within agreed protocols.
• Assist with medication reviews, flagging issues needing pharmacist or GP input.
• Support safe, evidence-based and cost-effective prescribing, including national and local guidance.
• Identify deprescribing and medication optimisation opportunities.
• Process repeat prescription requests and check them for appropriateness, with the Prescribing Clerks.
• Manage prescription synchronisation and medication queries, escalating complex ones.
• Support patients on repeat dispensing services.
• Carry out medicines reconciliation after hospital discharge, outpatient appointments and transfers, and resolve discrepancies.
• Escalate complex clinical concerns to the Clinical Pharmacist or GP.
• Use clinical systems to identify patients needing monitoring or review.
• Support prescribing audits, QAIF and other contract requirements.
• Extract and analyse prescribing and medicines-related data.
