Job summary

The Reddish Family Practice in Stockport is seeking an enthusiastic Pharmacy Technician to join a growing multidisciplinary team within general practice. You will support medicines optimisation, repeat prescribing, chronic disease management and medication safety with line management and support from our Clinical Lead Pharmacist Partner.

Based across five days with some hybrid working, you will handle medicines changes after hospital discharge, support repeat prescribing, undertake medicines

Qualifications

• Knowledge of medicines management and patient safety.
• Excellent communication and organisational skills.
• Positive attitude and ability to learn and adapt to new projects/skills.
• Reliable, personable and able to work as part of a busy multidisciplinary team to deadlines.

Responsibilities

• Process medication changes following hospital discharge communications.
• Support repeat prescribing and repeat dispensing systems.
• Undertake medicines reconciliation and medication reviews within competence.
• Respond to patient medication queries.
• Support chronic disease management and inhaler technique assessments.
• Maintain accurate patient records and undertake prescribing audits.
• Support annual vaccination programmes.
• Support with quality improvement projects and clinical audit.
