Job summary

Provide is seeking a Pharmacist to join its Pharmacy Team, driving medicines optimisation, patient safety and governance across community health services. You will work with multidisciplinary teams to shape medicines-related pathways and support innovative service developments.

Qualifications

Significant experience in Community Health Services is required.
Eligible for UK registration with current professional status.
Experience in medicines governance and risk management is desirable.

Responsibilities

Lead medicines optimisation and pharmaceutical processes across Provide services to ensure they are safe, effective, patient-centred, and evidence-based.
Provide expert clinical pharmacy advice to multidisciplinary teams, supporting safe management of medicines in community services.
Lead the pharmaceutical aspects of Sexual Health Services, including medicines governance and reporting.
Act as the professional lead for Non-Medical Prescribing, supporting governance, safe prescribing and service improvement.
Support identification and mitigation of medicines-related risks, contributing to patient safety and assurance.
Drive quality improvement initiatives, audits, and service evaluations to enhance medicines use and outcomes.
