Job description

We have an exciting opportunity to recruit an Intellectual disability nurse to our Primary care liaison Team.

We are looking for a motivated and enthusiastic RNLD who is passionate about supporting reasonable adjustments for patients with intellectual disabilities in the community.

We welcome applicants who are interested in supporting people with intellectual disability to enable them to have their physical health needs met within a primary care setting.

We support practices to keep a learning disability register and promote the health needs of the population.

We are looking for someone who is passionate about enabling our patients to access generic health screening and break down barriers in order to achieve this.

Responsibilities include:

  • Work with individuals to facilitate their access to primary health care services and liaise with other health professionals within multi-agency teams to advocate for service users.
  • Work with local GP Practices to help identify patients who are eligible for an annual health check and are flagged and encouraged to have an annual health check.
  • Deliver training around the Enhanced Service around health checks to primary care colleagues.
  • Work with the national screening programmes to support them to meet the needs of the local intellectual disability population around breaking down barriers to support access to screening.

The post holder will be committed to improving the life chances of people with intellectual disabilities.
