Job summary

Custom House Surgery in East London is seeking a Clinical Pharmacist to join a multidisciplinary primary care team. You will lead structured medication reviews, optimise medicines use, and support patients with long-term conditions while providing expert prescribing advice to GPs and staff.

You will review discharge information, manage repeat prescribing, and participate in audits, governance, and service improvement. A strong clinical background and independent prescribing are highly valued.

Qualifications

• Holds an MPharm degree and GPhC registration.
• Independent prescribing qualification preferred or essential.
• Experience in primary care pharmacy or medicines optimisation.

Responsibilities

• Conduct structured medication reviews and medicines optimisation clinics.
• Support patients with long-term conditions to ensure safe use of medicines.
• Review hospital discharge letters and reconcile medicines.
• Provide expert clinical advice on prescribing to GPs and clinicians.
• Identify prescribing risks and conduct audits.
