Job summary

West London NHS Trust is recruiting a pharmacist to join the Care Home GP Practice. You will work as part of a multidisciplinary team delivering enhanced primary care to nursing and residential homes across Ealing, with a focus on medication reviews and polypharmacy management.

The post involves collaboration with GPs, nurses, and care home staff, supporting prescribing, medicines reconciliation on transfer of care, and medicines optimisation as part of a transformation programme within Ealing

Qualifications

• Pharmacist to provide clinical medicines advice within a multidisciplinary team.
• Experience with polypharmacy in care home settings is preferred.
• Strong capability in medicines optimisation and patient care.

Responsibilities

• Undertake clinical medication reviews to manage polypharmacy in residents.
• Support repeat prescriptions and medicines reconciliation on transfers.
• Provide advice on clinical medicines issues to care home staff.
• Contribute to medicines optimisation and quality improvement initiatives.
