Job summary

Custom House Surgery in East London is seeking a Clinical Pharmacist to join a multidisciplinary primary care team. You will lead structured medication reviews, optimise medicines use, and support patients with long-term conditions while providing expert prescribing advice to GPs and staff.

You will review discharge information, manage repeat prescribing, and participate in audits, governance, and service improvement. A strong clinical background and independent prescribing are highly valued.

Qualifications

Join a forward-thinking, supportive GP practice in the heart of East London, where you'll play a key role in delivering high-quality patient care and improving medicines optimisation. Working as part of a multidisciplinary team, you'll use your clinical expertise to support patients with long-term conditions, conduct medication reviews, manage prescribing queries, and help reduce health inequalities within our diverse community. As a Clinical Pharmacist, you will work as part of a multidisciplinary primary care team to deliver high-quality, patient-centred care. Key responsibilities include:

• Conducting structured medication reviews and medicines optimisation clinics.
• Supporting patients with long-term conditions to ensure safe and effective use of medicines.
• Managing prescribing queries and providing expert clinical advice to GPs and other healthcare professionals.
• Reviewing hospital discharge letters and medicines reconciliation.
• Identifying and reducing prescribing risks, ensuring compliance with local and national guidance.
• Supporting the management of repeat prescribing systems and conducting audits.
• Contributing to the achievement of Quality and Outcomes Framework (QOF) and other contractual requirements.
• Providing patient education and promoting shared decision-making regarding treatment options.
• Working collaboratively with GPs, nurses, ANPs, HCAs and wider healthcare professionals to improve patient outcomes.
• Participating in practice meetings, service development and quality improvement initiatives.

Clinical Duties

• Undertake structured medication reviews and medicines optimisation for patients with complex needs.
• Manage patients with long-term conditions, ensuring medicines are used safely and effectively.
• Review and reconcile medicines following hospital discharge and outpatient appointments.
• Identify and manage prescribing risks, adverse drug reactions, and medication-related problems.
• Respond to prescribing and medicines-related queries from clinicians, patients, and carers.
• Support the management of repeat prescribing systems and processes.
• Provide patient-centred consultations, including face-to-face, telephone, and remote appointments.
• Promote shared decision-making and provide education to patients regarding their medicines.

Prescribing and Medicines Optimisation

• Optimise prescribing in line with national and local guidance.
• Support cost-effective prescribing and medicines management initiatives.
• Conduct audits and quality improvement projects relating to prescribing.
• Monitor high-risk medicines and ensure appropriate clinical monitoring is in place.
• Assist in achieving Quality and Outcomes Framework (QOF), Enhanced Services, and other contractual targets.

Multidisciplinary Working

• Work collaboratively with GPs, ANPs, Nurses, HCAs, Social Prescribers, and other healthcare professionals.
• Participate in multidisciplinary team meetings and case discussions.
• Develop and maintain effective working relationships with community pharmacists, care homes, secondary care colleagues, and other external stakeholders.

Governance and Quality Improvement

• Maintain accurate, timely, and comprehensive patient records.
• Support clinical governance, patient safety, and risk management activities.
• Participate in clinical audits, significant event reviews, and service improvement programmes.
• Ensure compliance with prescribing legislation, professional standards, and practice policies.

Professional Development

• Maintain professional registration and comply with Continuing Professional Development (CPD) requirements.
• Keep up to date with developments in primary care pharmacy and medicines management.
• Participate in mandatory training and practice development activities.

General Responsibilities

• Maintain patient confidentiality and comply with data protection requirements.
• Work in accordance with practice policies, NHS guidance, and professional standards.
• Promote equality, diversity, and inclusion in all aspects of care and employment.
• Support the wider objectives and development of the practice.
• Master's Degree in Pharmacy (MPharm) or equivalent.
• Registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Independent Prescriber qualification
• Demonstrable post-registration experience in primary care, general practice or medicines optimisation.
• Sound knowledge of current prescribing guidelines, medicines management, and clinical governance.
• Experience of conducting structured medication reviews and medicines reconciliation.
• Ability to manage patients with long-term conditions and optimise medication therapy.
• Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
• Strong clinical decision-making and problem-solving skills.
• Proficient in the use of clinical systems and Microsoft Office applications.
• Commitment to continuous professional development and evidence-based practice.
• Understanding of confidentiality, information governance, safeguarding, and equality and diversity requirements.
• Experience working within a GP practice or Primary Care Network (PCN).
• Experience supporting QOF, Enhanced Services, and medicines optimisation initiatives.
• Experience undertaking clinical audits, quality improvement projects, or service development work.
• Knowledge of EMIS Web clinical system.
• Experience working with diverse and multicultural patient populations.
• Leadership, mentoring, or supervisory experience.
• Membership of a relevant professional body or specialist pharmacy group.
