Job summary

Haslington Surgery is seeking a motivated Care Coordinator to support patient case management and liaison with community health services. You will work closely with clinical and admin teams to deliver coordinated care, including managing referrals, patient queries, and MDT coordination.

The role requires strong organisational and interpersonal skills, with experience in healthcare administration and IT systems. Hours are 24 hours per week across Tue–Fri with flexible workload expectations.

Qualifications

• NVQ Level 3 or equivalent knowledge in office procedures.
• Experience in a patient-focused environment.
• Experience with a wide range of administrative systems and software.
• Experience in planning and organising complex meetings/agendas.
• Experience using IT systems and data analysis.
• Excellent verbal and written communication skills.
• Ability to work on own initiative and manage workload to deadlines.
• Demonstrates cooperative teamwork and understanding of other professionals' roles.
• Commitment to equality and confidentiality.

Responsibilities

• Support admin team with urgent and non-urgent referrals.
• Manage patient home visit request book.
• Act as first point of contact for patient complaints.
• Prioritize clinical team workload with management.
• Use IT packages to contact patients.
• Process patient records.
• Arrange clinical meetings and provide minutes.
• Maintain patient information boards and screens.
• Coordinate and navigate across health and care services with link workers.
• Support MDT coordination.
