Milton Park Surgery, part of the Brunel Primary Care Network, is recruiting a Primary Care Coordinator to help manage proactive care for frail and housebound patients. You will maintain patient registers, coordinate Same Day Acute Visiting Service appointments, and contact patients by phone to arrange visits and follow-up care.

The role is multidisciplinary, offering training in SystmOne recording and coding, data extraction for reporting, and support for MDT/GSF meetings while safeguarding

Competenze

• GCSE English and Maths at A-C or equivalent.
• Previous admin or coordination experience.

Mansioni

• Maintain and manage registers of patients at increased risk, ensuring accuracy.
• Coordinate appointments, including the Same Day Acute Visiting Service.
• Contact patients and carers by telephone to arrange appointments, home visits and follow-up care.
• Coordinate and arrange GSF meetings, including scheduling, preparing patient lists and information for the clinical team.
• Support coordination of care for frail, housebound, palliative and clinically vulnerable patients.
• Support the organisation of long-term condition management, including patient lists, recalls, appointments and monitoring activity.
• Liaise with patients, carers, practice teams and wider healthcare professionals to support continuity of care.
• Record and follow up actions arising from GSF and multidisciplinary team meetings.
• Support accurate clinical coding and record keeping on SystmOne.
• Assist with collecting, extracting and collating clinical and service data for reporting.
• Assist with organising clinical activity to make effective use of appointments and resources.
• Maintain confidentiality and handle patient information in line with information governance requirements.
