• Clinically assess and treat patients using medicines expertise in specified disease areas.
• Prescribe and work with PCN and general practice team.

• Lead medicines optimisation and contribute to quality improvement and guidelines.
• Provide medication reviews during domiciliary visits and in practices.
• Support integration with wider health and social care teams to improve patient outcomes.
• Liaise with community and hospital pharmacists and other stakeholders in the medicines supply chain.
• Deliver long term condition clinics and adapt care for complex patients.
