Job summary

Marisco Medical Practice in Mablethorpe is seeking two experienced Practice Nurses with a Diploma in Diabetes to join a friendly multidisciplinary team. The roles include 30 hours per week and 20 hours per week, delivering high-quality diabetes care, immunisations, and chronic disease management.

You will work in a supportive environment with opportunities for professional development and a commitment to patient-centred care in a busy coastal practice.

Qualifications

• Registered Nurse with current NMC registration.
• Diploma in Diabetes Care.
• Experience in primary care.
• Competent in managing patients with diabetes, including initiating and reviewing treatment plans.
• Cervical cytology qualification and immunisation/vaccination training.

• Provide patient education and lifestyle advice.

Key responsibilities

• Deliver diabetes reviews and ongoing management.
• Provide patient education and lifestyle advice.
• Monitor and optimise diabetes treatment in line with national guidelines.
• Undertake chronic disease management clinics.
• Administer vaccines and immunisations.
• Perform cervical screening, wound care and health promotion activities as appropriate.
• Maintain accurate clinical records and contribute to quality improvement initiatives., Our multidisciplinary team includes experienced GPs, Advanced Clinical Practitioners, Practice Nurses, Nurse Associates, Healthcare Assistants, Phlebotomist, Clinical Pharmacists, First Contact Practitioners, and an excellent administrative and reception team. We are looking for two experienced, compassionate and motivated Practice Nurses with Diplomas in Diabetes, to join our friendly primary care team. The successful candidates will play a key role in delivering high-quality nursing care with a particular focus on diabetes management. There are two positions available: 1x 30 hours per week and 1 x 20 hours per week.

Requirements

• Ability to work independently and as part of a multidisciplinary
• team., Experience in managing other long term conditions (e.g. Asthma,
• COPD).
• Cervical cytology qualification.
• Immunisation and vaccination training.
• Travel health and wound care experience.
