Job summary

Holsworthy and Hatherleigh Medical Centres are seeking a Practice Nurse to deliver high-quality care across a broad range of services. You will perform duties typical of an RGN and contribute to travel health, immunisation, wound care and chronic disease management.

The role requires NMC registration, independent working ability, and strong IT skills to document care and coordinate with the GP team. Training and development are supported within a forward-looking practice.

Qualifications

  • NMC registration and up-to-date practice requirements.
  • Clinical skills in vaccination, immunisation and wound care.
  • Ability to work autonomously within a GP practice setting.
  • Familiar with primary care IT systems and health records.

Responsibilities

  • Provide travel health advice and vaccinations under policy guidance.
  • Carry out venepuncture and wound management per guidelines.
  • Manage chronic disease clinics and patient lifestyle advice.
  • Document consultations accurately and maintain patient confidentiality.
  • Collaborate with GPs, nurses and practice staff to ensure safe care.

Job description

As a Practice Nurse, you will provide high-quality nursing care to patients across a broad range of services, to include all duties normally undertaken by an RGN and in addition any duties as appropriate., Professional:

  • Practice independently in accordance with NMC code of conduct and with reference to practice guidelines, appropriately referring to GPs
  • Maintain accurate documentation of care and utilisation of IT
  • Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
  • Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including vaccinations and medicines, safe sex, food hygiene, sun protection. Refer to Travel Clinic if complex
  • Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions
  • Running chronic disease clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitates change
  • Assessing problems presented opportunistically by patients, dealing with minor injuries.
  • Able to document consultations according to NMC guidelines
  • Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse
  • Perform a holistic assessment of patients attending for ear care including irrigation
  • Perform a holistic assessment of patients attending for cervical cytology smear tests
  • Perform venepuncture according to local guidelines
  • Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines
  • Suture removal
  • Take ECGs
  • Assist in the provision of minor surgery
  • Ability to obtain and document informed consent (either verbal or written)
  • Ensure infection control guidelines are maintained
  • Ability to monitor and manage maintenance of stock and equipment to include refrigeration, sterilizer and emergency equipment
  • Confidentially of information gained at work must be preservedat all times
Managerial
  • Contribute to the assessment of service needs
  • Assist patients to identify their health needs
  • Contribute a nursing perspective to the practice development plan
  • Manage and organise individual patient consultations
  • Aware of identification and reporting procedures related to professional standards

  • Support the Practice clinical governance agenda
  • Identify changes to clinical practice that are required to implement evidence-based guidelines
Educational
  • Identify own personal development and training needs with line manager

Personnel
  • Take reasonable care of own safety and that of other persons who may be affected by own act or omission.
  • Co-operation with the practice to ensure all members of the team adheres to statutory regulations/policies and codes of practice and departments safety rules
Qualifications and Experience
  • Knowledge of patient group directions and associated policy
  • Clinical skills chronic disease management, cervical cytology, immunisation and vaccination
  • Change-management skills and ability to support patients to change lifestyle
  • Communication skills, both written and verbal
  • Ability to communicate difficult messages to patients and families
  • IT skills, competent with Microsoft Office 365, email, and use of internet as a resource, Understanding and knowledge of healthcare provision in GP practices, QOF and enhanced services
  • Awareness of local and national health policy
  • Knowledge of public health issues in the area
  • Knowledge of clinical governance issues in primary care
  • Audit skills
  • Knowledge of Primary Care IT systems i.e. SystmOne, Post registration/working towards experience in a primary care setting
  • Long-term condition management
  • Experience working autonomously and as part of a team, Independent/supplementary nurse prescribing qualification
  • Minor injuries qualification
  • Chronic Disease management qualification
