Job summary

Great North Medical Group in Doncaster seeks a dedicated Practice Nurse to join a supportive team. The post involves providing high-quality nursing care, managing chronic diseases, vaccinations, wound care, and health promotion within the primary care setting.

You will work with GPs, healthcare assistants and the wider team to deliver safe, patient-centred services and maintain accurate records while contributing to ongoing service development.

Qualifications

  • Registered Nurse (Adult) with current NMC registration.
  • Experience of working in primary care, community or general practice setting.
  • Experience delivering treatment room services and routine nursing procedures.
  • Experience managing patients with diabetes.
  • Competent in vaccinations and immunisations.
  • Experience venepuncture, wound care and cervical cytology screening.

Responsibilities

  • Deliver high quality nursing care within own scope of practice.
  • Undertake chronic disease management reviews per protocols and national guidance.
  • Provide health promotion and lifestyle advice to patients.
  • Administer vaccinations and immunisations per guidance.
  • Undertake cervical cytology screening.
  • Carry out wound care, dressings and suture/clip removal.
  • Perform routine treatment room procedures including BP, ECGs and venepuncture.
  • Support infection prevention and control procedures.
  • Maintain accurate clinical records.
  • Participate in audits and quality improvement initiatives.
  • Work with multidisciplinary team to support patient care.
  • Identify and report safeguarding concerns.
  • Contribute to QOF targets and contractual requirements.
  • Maintain professional registration and CPD.

Job Summary

An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our friendly and supportive team at Great North Medical Group.

The successful candidate will provide high quality, evidence based nursing care to our practice population and work collaboratively with GPs, healthcare assistants and the wider multidisciplinary team to deliver safe, effective and patient centred services.

The role will involve providing a range of treatment room and chronic disease management services, supporting health promotion initiatives and contributing to the ongoing development of clinical services within the practice.

Main Duties and Responsibilities
  • Deliver high quality nursing care within own scope of practice.
  • Undertake chronic disease management reviews in line with practice protocols and national guidance.
  • Provide health promotion and lifestyle advice to patients.
  • Administer vaccinations and immunisations in accordance with national and local guidance.
  • Undertake cervical cytology screening.
  • Carry out wound care, dressings and removal of sutures and clips.
  • Perform routine treatment room procedures including blood pressure monitoring, ECGs, venepuncture and other relevant clinical interventions.
  • Support infection prevention and control procedures.
  • Maintain accurate, contemporaneous and complete clinical records.
  • Participate in clinical audits, quality improvement initiatives and practice meetings.
  • Work collaboratively with the wider multidisciplinary team to support patient care.
  • Identify and report safeguarding concerns in accordance with practice policies.
  • Contribute to the achievement of Quality and Outcomes Framework (QOF) targets and other contractual requirements.
  • Maintain professional registration and undertake continuing professional development.

Essential
  • Registered Nurse (Adult) with current NMC registration.
  • Experience of working within a primary care, community or general practice setting.
  • Experience of delivering treatment room services and routine nursing procedures.
  • Experience of managing and supporting patients with diabetes.
  • Experience of providing health promotion and preventative healthcare advice.
  • Competent in administering vaccinations and immunisations.
  • Experience of venepuncture, wound care and cervical cytology screening.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Good organisational skills and ability to prioritise workload effectively.
  • Competent IT and clinical record keeping skills.
  • Commitment to maintaining professional development and evidence based practice.
  • Understanding of safeguarding responsibilities and patient confidentiality.
  • Flexible, reliable and committed to delivering high quality patient care.
Desirable
  • Qualification in Diabetes Care or Chronic Disease Management.
  • Experience of insulin initiation and diabetes reviews.
  • Experience of asthma and COPD management.
  • Experience of using SystmOne.
  • Independent Prescribing Qualification.
  • Experience working within General Practice.
