Job summary

Meadowgreen Health Centre is seeking a Practice Nurse to join the nursing team across two GP site with around 19,000 patients. The role covers assessment, treatment and management of acute, chronic and complex conditions, including immunisations, cervical screening, wound care and medication reviews.

Newly registered or experienced nurses with NMC registration are welcome. The post offers full-time with flexible/compressed hours and scope to take ownership of a clinical area.

Qualifications

• Evidence of ongoing CPD relevant to the role.
• Registration with the Nursing and Midwifery Council (NMC).

Responsibilities

• Assess, plan, deliver and evaluate care for patients with acute, chronic and complex health needs.
• Undertake health promotion, screening, immunisations, chronic disease monitoring, wound care and medication reviews within scope of practice.
• Support patients to improve their health outcomes.
• Recognise urgent clinical situations and respond, referring when required.
• Communicate with patients, carers and healthcare professionals to coordinate care.
• Maintain accurate clinical records and practise to NMC standards, national guidance and practice policies.
• Contribute to audits, quality improvement, safeguarding, infection prevention and risk management.
• Liaise with community and specialist services to support continuity of care.
• Maintain professional development and support the learning of colleagues and students.
