Job summary

The Lighthouse Group Practice is seeking a Practice Nurse to join its nursing team at a busy inner-city GP surgery. You will focus on patient assessment, chronic disease clinics, immunisations and prevention across all ages.

Applicants need current NMC registration as an RGN and prior primary care practice nursing experience to support GPs and deliver high-quality care. You will also contribute to wound care, cervical screening, travel health and minor surgery support.

Qualifications

• Current NMC registration.
• Experience working as a practice nurse in a primary care setting.

Responsibilities

• Assess, plan and evaluate care for patients with acute and long-term conditions.
• Run chronic disease clinics for conditions such as diabetes, asthma, COPD, hypertension and CHD.
• Give immunisations and vaccinations in line with national schedules, and travel health advice.
• Provide wound care, including leg ulcer management and Doppler assessments.
• Deliver cervical screening where trained and accredited.
• Support minor surgery and post-operative care.
• Request pathology tests and interpret results under protocols.
• Review medication compliance and concordance, and promote medicines optimisation.
• Give lifestyle advice and health promotion on smoking, weight and alcohol.
• Keep SNOMED CT clinical records and disease registers, and follow infection control and safeguarding policies.
• Recognise and respond to clinical emergencies.
• Refer patients to secondary care and community services.
