Job summary

Kent Local Medical Committee is seeking a part-time practice nurse to work 16 hours per week in Tonbridge. The role focuses on delivering high-quality nursing care and supporting the health and wellbeing of the practice population.

You will manage chronic conditions, provide screenings, immunisations, travel health advice and wound care, and support patients with lifestyle changes and medication management. You will also maintain records and contribute to quality improvement.

Qualifications

• Registered nurse with current UK NMC registration.
• Experience delivering nursing care in primary care or general practice.
• Experience with long-term conditions management.

Responsibilities

• Deliver evidence-based nursing care within the practice setting.
• Managing and supporting patients with long-term conditions including diabetes, asthma, COPD, hypertension, and cardiovascular disease.
• Undertaking chronic disease reviews.
• Providing cervical screening, immunisations, vaccinations, travel health advice, and wound care services.
• Supporting patients with lifestyle changes and delivering health promotion and wellbeing initiatives.
• Identifying patients at risk of developing long-term conditions and providing proactive interventions.
• Supporting patients with medication management and self-care.
• Maintaining accurate clinical records and contributing to quality improvement programmes.
• Working collaboratively with the wider multidisciplinary team and external healthcare professionals to ensure coordinated patient care.
• Supporting safeguarding, infection prevention, and clinical governance activities within the practice.
