Job summary

Chase Medical is supporting a well-established GP practice in DE12, near Ashby-de-la-Zouch, to recruit a confident and autonomous Practice Nurse. The role is part-time, 30 hours across 3 long days, with on-site parking and a local multidisciplinary team.

You will deliver chronic disease reviews, immunisations and wound care in a supportive environment. The ideal candidate will have experience in long-term condition management and enhanced skills in asthma or diabetes.

Qualifications

• Experience in chronic disease management.
• Ability to manage patients with long-term conditions.
• Vaccination and immunisation delivery.
• Dressing and wound care skills.
• Knowledge of QOF and primary care processes.
• Enhanced skills in asthma or diabetes (accredited).

Responsibilities

• Provide chronic disease reviews and immunisations.
• Deliver patient-centred nursing care in a GP practice.
• Work autonomously within a multidisciplinary team.

Job description

Salary: £21.00-£23.00+ per hour (depending on experience and skillset)

Hours: Part-time – 3 long days, ideally 30 hours

We're working with a well-established GP practice in DE12, CQC rated 'good', near Ashby-de-la-Zouch, looking for a confident and autonomous Practice Nurse to join their friendly clinical team.

Supporting a patient population of around 16,000, you'll work alongside 5+ GP Partners, an ANP, 2 Physician Assistant, 2 Practice Nurses, a Nurse Associate and 3 HCAs. This is a great opportunity for a nurse who enjoys working independently while being part of a well-supported multidisciplinary team.
