This role has been advertised by Hartlepool & Stockton Health on behalf of our member practice Densham Surgery. This exciting new role will provide safe, high-quality primary care nursing services to the practice population. Working within the NMC Code of Conduct, the post holder will work as part of the nursing team, using previous primary care experience to provide nurse interventions.

In addition to the main duties listed, the role involves quality assurance, service support, participation in quality improvement activities such as audits, significant event analysis, and development of protocols and new services. Recognise opportunities to reduce inequalities and unwarranted healthcare variations. Undertake mentorship for preceptor programmes and contribute to revalidation of nurse colleagues. Analyze data to improve screening and immunisation programme uptake, including coordinating vaccination campaigns. Complete statutory and skills-specific training, participate in annual performance reviews, and maintain a record of personal and professional development. Take responsibility for your own learning, identify knowledge gaps, and pursue further learning to enhance nursing services.
