Job description

Are you a qualified and dedicated Practice Nurse looking to make a meaningful impact in primary care? We are excited to offer an opportunity for you to join our dynamic team at Southwell Medical Centre.

Within this role, you will deliver high-quality, evidence-based nursing care to patients across all age groups within the general practice setting.

Main duties of the job

We are looking for an experienced practice nurse who can undertake all practice nursing duties and act as the respiratory lead within the nursing team, looking after our more complicated asthma and COPD patients alongside the GPs.

The right candidate would ideally already have experience in managing respiratory illness or must have a willingness to learn.

Southwell Medical Centre is a semi-rural practice in the heart of Nottinghamshire, serving a patient population of circa 12300 patients.

We have a large multi-disciplinary clinical team including permanently employed staff as well as ARRS staff employed by our PCN.

We are a really friendly team, looking to maintain good levels of patient care within our nursing team.

We offer enrolment into the NHS Pension Scheme, have on-site parking and also offer an employee assistance programme.

Job responsibilities

Key ResponsibilitiesClinical Responsibilities

  • Conduct chronic disease management clinics including asthma, COPD, diabetes, heart failure, CHD
  • Administer routine immunisations, including childhood immunisations, travel vaccines and seasonal vaccinations, adhering to national guidelines
  • Perform cervical screening and manage contraception reviews
  • Undertake dementia and learning disability reviews, offering support and guidance to caregivers
  • Engage in health promotion activities, educating patients on lifestyle modifications to prevent disease and promote wellbeing
  • Provide wound management including dressings and suture removal, ensuring timely and effective healing
  • Collaborate with GPs and other healthcare professionals to deliver integrated and patient focussed care
  • Maintain accurate clinical records, ensuring compliance with data protection and confidentiality standards
  • Contribute to the development and implementation of practice policies and protocols, enhancing service delivery and patient outcomes
  • Provide guidance and support to the leadership team and wider practice for infection prevention control
Person Specification
Qualifications
  • You must be a Registered General Nurse (RGN) with a valid NMC registration.
  • You must be able to evidence experience or formal training in general practice nursing, primary care or community health.
  • You must demonstrate regular CPD in line with NMC revalidation requirements.
