We are a rapidly growing GP-led organisation with a mission to ensure that all patients have access to comprehensive, coordinated and continuous care within a sustainable primary care system.
We acknowledge that our primary care system is under pressure, but we believe that we can turn this around by reimagining how primary care is delivered from the ground up by placing a focus on data insights, technology and clinical innovation. Ultimately, we want to improve patient access, patient experience and health outcomes to significantly increase the quality of care for our patients.
The Much Birch GMS contract was novated to the Federation in February 2024, but the practice team remains unchanged as does the identity of the surgery in the local community.
Much Birch is :-
A supportive clinical team of GPs, ANPs, Paramedics, Practice Nurses, HCAs, Physiotherapist, Social Prescribers.
Thepostholder will utilise general practice nursing skills to deliver primarymedical services to Much Birch population. They will provide highquality general practice nursing care as an autonomous practitioner, supportothers in the clinical area and work within their scope of practice.
Thepostholder will demonstrate an enhanced range of skills and experienceas outlined in the Reporttemplate Capability Framework -Skills for Health 2021- and as aPractice Nurse (Enhanced Nurse Level) will deliver the fundamental capabilitiesof General Practice Nursing.
Role Core Competencies
Willrequire a critical understanding of detailed theoretical and practicalknowledge, which can be generalist with a special interest and/or havemanagement and leadership responsibilities.
Havespecialist knowledge, skills, and capabilities.
Consultwith patients, their family, and the multi-professional team to undertakeassessments of patient need and devise and evaluate complex care plans.
Evaluateand analyse clinical problems using their clinical knowledge, seeking out andapplying relevant evidence, enhanced techniques, interventions, and equipmentto make clinical decisions.
Deliverenhanced clinical care in the context of continual change, challengingenvironments, different models of care delivery, innovation and rapidlyevolving technologies using analysis and their underpinning knowledge to managecomplex interventions.
Teach andadvise patients and their families on how to manage their condition or supportthe multi-disciplinary team to do so.
Participatein clinical audits and research projects, and implement changes as required,including the development, and updating of practice protocols / guidelines andprocedures locally.
Workwithin national and local protocols where these exist.
Recogniseboundaries of their practice and know when and to whom patients should bereferred.
Delegatework to other members of the multidisciplinary team and take accountability forthe delegated activity.
Demonstrateinitiative and be creative in finding solutions to problems.
Responsibilityfor team performance and service development and consistently undertakeself-development.
The role will also include core expected patientactivities and to oversee and educate staff to deliver and monitor the qualityof nursing delivered. This will include, but is not limited to:
Simple and complex wound management, doppler and compression
Provision, ordering and safe storage of vaccines for all ages inline with national guidance and an up to date knowledge of PGDs.
Administering routine injections such as B12 and decapeptyl asprescribed
Undertakingcervical cytology screening
Contraceptioncare and sexual health assessment and signposting
Regularchecks for people with long term conditions this may include diabetes, CHD,heart failure, stroke / TIA, PAD, hypertension, CKD, respiratory care andlearning disabilities.
Supervisionof junior clinical staff and students
Toact as a link worker for an identified population of patients for example thosewith an identified learning disability.
