Job Responsibilities

The Practice Nurse will be responsible for implementing processes for effective management of patients with long-term conditions, including elderly and housebound patients, using evidence-based practices. As a senior member of the multidisciplinary team, the nurse will ensure the delivery of nursing services to the entitled patient population. Key areas of responsibility include:

  1. Infection prevention and control
  2. Health promotion
  3. Chronic disease management (e.g., diabetes, hypertension, respiratory conditions)
  4. Well women and well men clinics
  5. Supporting practice management in reviewing and delivering clinical policies and procedures
Minimum Requirements
  • NMC Registration
  • Experience in chronic disease management, including diabetes, hypertension, and respiratory conditions
  • Previous practice nurse experience within primary care
  • Qualified smear taker
  • Immunization/vaccination trained, including childhood immunizations
