Practice Nurse Position at Auchtermuchty Health Centre

An exciting opportunity has arisen at Auchtermuchty Health Centre for a Practice Nurse with experience in chronic disease management.

We are seeking an individual skilled in managing one or more long-term health conditions, which could include:

• Diabetes
• Asthma
• Cardiovascular diseases

Hours and working patterns are flexible to suit both parties, and pay will be commensurate with skill and experience.

Key Responsibilities:

1. Long-term management of patients with chronic disease through clinic reviews.
2. Reviewing patient health questionnaires and test results.
3. Identifying and managing patients who would benefit from health optimization through education, lifestyle changes, and medication.
4. Responsible for stock ordering and replenishment.
5. Core nursing skills such as wound care, phlebotomy, ECG taking, injections, and cervical screening are essential. Note that some of these services are now provided by CTAC colleagues.
