Job summary

We are looking for a Clinical Pharmacist to join our established Clinical Team.

Applicants must have a passion for providing excellent, professional, and effective care whilst ensuring our patients feel safe and respected.

Main duties of the job

1. Manage repeat prescription authorisations and reauthorisation, acute prescription requests, management of medicines on transfer of care, and systems for safer prescribing.
2. Perform medication reviews of patients with polypharmacy, especially those with co-morbidities.
3. Develop and manage medicines optimisation services.
4. Develop and manage a medicines management plan and deliver patient services as per practice policy and guidance.
5. Lead and develop the medicines management team, including training, to promote cost-effective prescribing and enhance patient care.

Broadway Medical Centre is a forward-thinking, patient-focused, professional, and friendly team caring for approximately 10,300 patients in Fleetwood.

Our team includes 2 GP Partners, 4 Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Healthcare Assistants, and administrative staff. We are a training practice for GPs and part of the Fleetwood Primary Care Network, collaborating with Clinical Pharmacists, Pharmacy Technicians, Paramedics, Mental Health Team, and Social Prescribers.
