We Are Hiring

• Department: Ascension Medical Group
• Schedule: Full-Time, Days Mon. - Fri. 8 AM - 5 PM
• Hospital: Ascension St. Vincent's
• Location: 4500 Salisbury Road, Ste 210 Jacksonville, Florida 32216-0968, United States

What You Will Do

Coordinate and identify care needs (gaps in care) for patients attributed to a primary care practice.

• Identify patients through health plans' gaps in care list, immunization databases, medical records, and patient disease management registries.
• Collaborate with providers to review care needs and validate these needs against medical records.
• Reach out to patients with validated gaps in care to facilitate meeting these needs.
• Support quality outcomes and high reliability in the Patient-Centered Medical Home.

What You Will Need

Education:

• High School diploma or equivalency OR 1 year of relevant job-specific experience.
• Note: Professional licensure/certification can be used in lieu of education or experience, if applicable.
