Role Summary
The HOSM-SAS Cluster incorporates Seaton, Axminster and Sidmouth (SAS), Honiton and Ottery St Mary (HOSM). It has an attractive mix of rural villages and coastal towns, with excellent rail links to London and Exeter. The area is part of the Jurassic Coast World Heritage Site, ideal for coastal walks, fishing harbours, and beaches. The location offers flexible working opportunities and free parking, supporting a healthy work-life balance.
About the Role
We are seeking a Band 6 Specialist Physiotherapist to join the team in an integrated rehabilitation and urgent care pathway across community services in HOSM. The role offers support for skill development, CPD opportunities, and autonomy in practice. You will work alongside experienced professionals including Occupational Therapists, Physiotherapists, Rehab Nurses, and Support Workers.
The candidate should have an interest in community rehabilitation, multidisciplinary working, and managing patients with long-term conditions such as neurological, orthopedic, dementia, and falls prevention. The role emphasizes maximizing patient independence and quality of life, preventing hospital admissions, and supporting discharge processes. A car and valid driving license are required.
Working Pattern:
The role involves working alongside Urgent Community Response in HOSM on the Ageing Well rota, with shifts over seven days and flexible hours.
