Job summary

sjb medical is seeking a Band 5 Physiotherapist to join a multidisciplinary rehabilitation team in an intermediate care setting in Ware, Hertfordshire. You will manage a caseload of residents, conduct comprehensive physiotherapy assessments, and develop personalised treatment plans to improve mobility and independence.

Qualifications

• Degree in Physiotherapy and HCPC registration are essential.
• Experience working with older adults or rehabilitation services is desirable.
• Strong communication and interpersonal skills are required.

Responsibilities

• Manage a caseload of residents requiring rehabilitation support.
• Conduct comprehensive physiotherapy assessments and develop personalised treatment plans.
• Deliver evidence-based rehabilitation programmes focused on mobility, strength, and independence.
• Work with residents to set meaningful and achievable rehabilitation goals.
• Support discharge planning and safe transitions back into the community.
• Collaborate with Occupational Therapists, Social Workers, Doctors, and wider teams.
• Provide guidance and supervision to therapy assistants.
• Participate in multidisciplinary meetings and case reviews.
