Job summary

HCRG Care Group is seeking a Band 5 Physiotherapist to join our Intermediate Care Rehabilitation Service in Luton. You will assess and treat patients in their homes and in rehabilitation beds, managing your own caseload within a supportive multidisciplinary team.

You will complete holistic assessments, plan and deliver rehab interventions, monitor progress and adapt plans to maximise independence. A UK driving licence and car access are required to visit patients at home as part of the role.

Qualifications

• Diploma or BSc in Physiotherapy
• Current HCPC registration as a Physiotherapist
• Evidence of continuing professional development
• Knowledge and experience relevant to rehabilitation
• Good physiotherapy assessment and treatment skills
• Ability to establish appropriate functional rehabilitation goals
• Understanding of the principles of Intermediate Care
• Knowledge of safeguarding, clinical governance and professional boundaries
• Strong written and verbal communication skills
• Ability to communicate effectively with patients, families, carers and healthcare professionals
• Good organisation and time-management skills
• Ability to prioritise and manage your own caseload
• Confidence using IT systems and producing appropriate clinical documentation and reports
• Flexible and collaborative approach to working in a multidisciplinary team
• Commitment to ongoing professional and clinical development

Responsibilities

• Complete patient-centred and holistic physiotherapy assessments
• Work with patients to establish meaningful and achievable rehabilitation goals
• Plan, deliver and evaluate individualised physiotherapy interventions
• Manage and prioritise your own clinical caseload
• Work with patients in their homes and rehabilitation settings
• Monitor patient progress and adapt treatment programmes
• Respond to urgent referrals
• Provide clear advice and guidance to patients, families and other professionals
• Maintain clear, accurate and timely clinical records and reports
• Participate in multidisciplinary meetings and audits
• Support students as your experience develops
