disAbility Cornwall & Isles of Scilly is seeking a Personalised Support Specialist to help adults in North Cornwall live independently by providing tailored support and practical assistance. You will conduct home visits, perform holistic assessments, and coordinate housing, welfare benefits, and local services to improve wellbeing.
The role focuses on holistic support, client independence, and safety, with 37 hours per week, Monday to Thursday 9am–5pm and Friday 9am–4:30pm.

To provide person-centred support to people across North Cornwall, enabling them to live independently and recover confidence at home and in the community. This role focuses on holistic support, helping clients address both immediate needs and longer-term challenges to enhance their wellbeing. This role does not provide personal care.
Humans Cornwall is an independent service under the umbrella of disAbility Cornwall & Isles of Scilly. It works within the health system in collaboration with various partners to support people at risk of hospital admission, or upon their discharge. We deliver personalised and holistic support at this time in conjunction with internal and external providers.
