Job summary

FedBucks Ltd is seeking an enthusiastic Social Prescriber to join our multidisciplinary team across Buckinghamshire. The role focuses on personalised support for people with long-term conditions, frailty, social isolation and complex needs, helping them access community services and improve wellbeing.

The post involves coordinating care with GP practices, pharmacists, care coordinators and others to ensure timely, person-centred support and reduced reliance on hospital services.

Qualifications

• Ability to listen, empathise and provide person-centred support in a non-judgemental way.
• Understanding of wider determinants of health and community factors.

Responsibilities

• Provide personalised, holistic support to people with long-term conditions to access community services.
• Coordinate input from different professionals and support holistic care planning.
• Inform patients about services to promote health and independence.
• Reduce demand on statutory services to prevent avoidable hospital admissions and GP referrals.
