4Recruitment Services is seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Manchester/Trafford. You will support Children Looked After and Care Leavers aged 16–25 in health, education, housing and finances, helping them progress toward independent adulthood.

Qualifications

• Experience working with vulnerable young people aged 16–25.
• NVQ level 3 or equivalent qualification.
• Knowledge of Children Act 1989/2004 and Leaving Care guidance.
• Ability to manage challenging behaviour and develop strategies.
• Strong interpersonal skills with families, carers and professionals.
• Enhanced DBS clearance.

Responsibilities

• Collaborate with partner organisations to assess, plan and support care leavers.
• Manage a caseload of young people per policies and regs.
• Attend reviews and meetings for young people wherever they live.
• Assess needs and deliver intervention programmes for care leavers.
• Assess financial needs and help plan budgets for accommodation and maintenance.
• Maintain records and complete pathway planning meetings.
• Visit young people at home or other venues.

4Recruitment Services are seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Manchester/Trafford.

The role of the Personal Advisor is to support Children Looked After and Care Leavers in their transition to adulthood. You will work with young people aged 16-25 providing support in various areas including physical and emotional health, education, employment and training, independence skills, housing and finances. You will be responsible for working with young people in completing their pathway plan.

DUTIES & RESPONSIBILITIES INCLUDE:

• To work collaboratively with a range of partner organisations to assess, plan and support care leavers and their families.
• To manage a caseload of individual young people in accordance with departmental policies and procedures and relevant legislation, guidance and regulations.
• To attend and contribute to reviews and participate in meetings, wherever a young person is living
• To assess need and deliver support and programmes of intervention to a diverse group of young people leaving care in line with Departmental standards
• To assess financial needs of care leavers and assist them in planning their individual budgets for accommodation and maintenance.
• To undertake visits, complete assessments and reviews, maintain records in accordance with policy and procedures
• To be responsible for planning and recording Pathway Planning meeting where required.
• To visit young people at home and other external venues in accordance with standards, policy and procedures

ESSENTIAL REQUIREMENTS:

• Experience working with vulnerable young people aged 16 – 25
• Relevant qualification NVQ level 3 or equivalent.
• Knowledge and understanding of human growth and development and the particular issues relating to young people living in care
• Relevant legislation, guidance including Children Act 1989,2004, Children (Leaving Care) Act 2000 Regulations and Guidance.
• Ability to manage and develop effective strategies when working with young people presenting challenging behaviour.
• Strong interpersonal skills with a range of people including young people and their families, service providers, colleagues and managers.
