Job summary

4Recruitment Services are seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Bury, Greater Manchester. You will support Children Looked After and Care Leavers aged 16–25 across health, education, employment, independence, housing and finances, helping them with their pathway plan.

Responsibilities include collaborating with partners, caseload management, attending reviews, and delivering targeted interventions while ensuring records and DBS checks are up to date.

Qualifications

• Experience with vulnerable young people aged 16–25.
• NVQ Level 3 or equivalent qualification.
• Knowledge of growth and development of looked-after young people.
• Awareness of Children Act 1989/2004 and Leaving Care Act 2000.
• Ability to develop strategies when dealing with challenging behaviour.
• Strong interpersonal skills with young people, families and professionals.
• Enhanced DBS clearance.

Responsibilities

• Manage a caseload in line with policies and regulations.
• Attend and contribute to reviews and meetings for each young person.
• Assess needs and deliver programmes of intervention for care leavers.
• Assess financial needs and assist with pathway budgets for accommodation and maintenance.
• Visit clients and maintain records per policy and procedures.
• Plan and record Pathway Planning meetings when required.
• Visit young people at home or other venues as needed.

4Recruitment Services are seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Bury, Greater Manchester.

The role of the Personal Advisor is to support Children Looked After and Care Leavers in their transition to adulthood. You will work with young people aged 16-25 providing support in various areas including physical and emotional health, education, employment and training, independence skills, housing and finances. You will be responsible for working with young people in completing their pathway plan.

DUTIES & RESPONSIBILITIES INCLUDE

• To work collaboratively with a range of partner organisations to assess, plan and support care leavers and their families.
• To manage a caseload of individual young people in accordance with departmental policies and procedures and relevant legislation, guidance and regulations.
• To attend and contribute to reviews and participate in meetings, wherever a young person is living.
• To assess need and deliver support and programmes of intervention to a diverse group of young people leaving care in line with Departmental standards.
• To assess financial needs of care leavers and assist them in planning their individual budgets for accommodation and maintenance.
• To undertake visits, complete assessments and reviews, maintain records in accordance with policy and procedures.
• To be responsible for planning and recording Pathway Planning meeting where required.
• To visit young people at home and other external venues in accordance with standards, policy and procedures.

ESSENTIAL REQUIREMENTS

• Experience working with vulnerable young people aged 16-25
• Relevant qualification NVQ level 3 or equivalent.
• Knowledge and understanding of human growth and development and the particular issues relating to young people living in care
• Relevant legislation, guidance including Children Act 1989,2004, Children (Leaving Care) Act 2000 Regulations and Guidance.
• Ability to manage and develop effective strategies when working with young people presenting challenging behaviour.
• Strong interpersonal skills with a range of people including young people and their families, service providers, colleagues and managers.
• Enhanced DBS
