Job summary

4Recruitment Services are seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Bury, Greater Manchester. You will support Children Looked After and Care Leavers in their transition to adulthood, working with young people 16-25 across health, education, housing and finances.

You will be responsible for working with young people in completing their pathway plan. The role involves managing pathways, visiting young people at home or external venues, and collaborating with partner

Qualifications

• Experience working with vulnerable young people aged 16-25.
• Relevant qualification NVQ level 3 or equivalent.
• Knowledge of human growth and development and issues facing care leavers.
• Knowledge of Children Act 1989/2004 and Leaving Care Act 2000 regulations.
• Ability to develop strategies for young people with challenging behaviour.
• Strong interpersonal skills with young people, families, service providers and colleagues.
• Enhanced DBS clearance.

Responsibilities

• Collaborate with partner organisations to assess, plan and support care leavers and families.
• Manage a caseload of individual young people per policies, procedures and regulations.
• Attend and contribute to reviews and meetings wherever a young person is living.
• Assess needs and deliver interventions to diverse care leavers in line with standards.
• Assess financial needs of care leavers and help plan budgets for accommodation and maintenance.
• Undertake visits, complete assessments and reviews, maintain records per policy.
• Plan and record Pathway Planning meetings when required.
• Visit young people at home and other external venues per standards.

The role of the Personal Advisor is to support Children Looked After and Care Leavers in their transition to adulthood. You will work with young people aged 16-25 providing support in various areas including physical and emotional health, education, employment and training, independence skills, housing and finances. You will be responsible for working with young people in completing their pathway plan.

DUTIES & RESPONSIBILITIES INCLUDE:

• To work collaboratively with a range of partner organisations to assess, plan and support care leavers and their families.
• To manage a caseload of individual young people in accordance with departmental policies and procedures and relevant legislation, guidance and regulations.
• To attend and contribute to reviews and participate in meetings, wherever a young person is living.
• To assess need and deliver support and programmes of intervention to a diverse group of young people leaving care in line with Departmental standards.
• To assess financial needs of care leavers and assist them in planning their individual budgets for accommodation and maintenance.
• To undertake visits, complete assessments and reviews, maintain records in accordance with policy and procedures.
• To be responsible for planning and recording Pathway Planning meeting where required.
• To visit young people at home and other external venues in accordance with standards, policy and procedures.

ESSENTIAL REQUIREMENTS:

• Experience working with vulnerable young people aged 16 - 25
• Relevant qualification NVQ level 3 or equivalent.
• Knowledge and understanding of human growth and development and the particular issues relating to young people living in care
• Relevant legislation, guidance including Children Act 1989,2004, Children (Leaving Care) Act 2000 Regulations and Guidance.
• Ability to manage and develop effective strategies when working with young people presenting challenging behaviour.
• Strong interpersonal skills with a range of people including young people and their families, service providers, colleagues and managers.
