Job summary

Oxfordshire County Council is seeking a Personal Advisor for the Leaving Care Service. You will support care-experienced young people aged 16 to 25 as they transition to adulthood, providing guidance, advocacy and practical support at every stage of their journey.

You will develop Pathway Plans, coordinate housing, education, employment and health services, and work with partner agencies to ensure positive life outcomes for young people.

Qualifications

• Excellent communication and interpersonal skills.
• Experience in social care, youth work, education, housing, criminal justice or support services.

Responsibilities

• Support care-experienced young people aged 16–25 to navigate the transition to adulthood.
• Develop and review Pathway Plans reflecting ambitions and aspirations.
• Coordinate access to housing, education, employment, training and health services.
• Manage your own caseload and liaise with partner agencies across Oxfordshire.
• Advocate for young people to improve life chances and outcomes.
