Job summary

NHS Dumfries & Galloway is seeking a motivated Community Occupational Therapist to join the Stewartry Home Team at Gardenhill Health Centre. You will support adults to maximise independence and quality of life, working in the community to assess needs and develop person-centred intervention plans.

Role is based in Gardenhill Health Centre with a Band 6 salary (£43,231 - £52,679) and permanent employment. Applications welcome for part-time or full-time hours.

Qualifications

• Degree or Diploma in Occupational Therapy.
• Registration with HCPC.
• Excellent assessment, communication, and problem-solving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Full UK driving licence / ability to travel around the region independently.

Responsibilities

• Contribute to Discharge to Assess pathways for timely hospital discharges and community support.
• Deliver rapid response elements to prevent unnecessary admissions.
• Assess occupational performance, mobility, equipment, and environmental needs.
• Collaborate with patients, families, carers, health and social care professionals, and third-sector partners to plan care.
• Promote independence and quality of life via personalised interventions.
• Support anticipatory care planning and admission avoidance in the community.
