Job summary

HCRG Care Group in Bath & North East Somerset, Swindon and Wiltshire is seeking a Clinical Triage Practitioner to support Children’s Specialist Services. You will triage referrals, including urgent cases, ensuring timely assessment and communication of outcomes, within a multidisciplinary team and integrated care pathways.

The role involves safeguarding, care coordination and partnership working with health, education and third-sector partners.

Qualifications

• Registered Nurse or Allied Health Professional with current NMC/HCPC registration.
• Evidence of continuing professional development.
• Evidence of specialist clinical knowledge relevant to the role.
• Minimum 3 years' post-registration experience.
• Knowledge of safeguarding and child protection procedures.
• Experience with child and family-centred care.
• Demonstrable clinical engagement and assessment skills.
• Experience working with partner agencies and multidisciplinary teams.
• Ability to travel throughout Wiltshire.

Responsibilities

• Undertake clinical triage of referrals into Children's Specialist Services and School Aged Health Services in accordance with agreed pathways and timescales.
• Support the Clinical Triage Lead in the delivery of multidisciplinary triage for complex and multiple-need referrals.
• Provide specialist clinical advice and support to Care Coordination and single Point of Access functions.
• Ensure children, young people and families are directed to appropriate services through effective care navigation.
• Communicate complex, sensitive and potentially distressing information to families and professionals as required.
• Work collaboratively with health, education, social care and third-sector organisations to ensure coordinated care delivery.
• Identify, elevate and manage safeguarding concerns, clinical risks and pathway issues.
• Contribute to service development, governance processes, implementation of standard operating procedures.
