Job description

One Ellesmere Port Primary Care Network (PCN) is a collaboration of the 6 GP Practices from across the town. Working together at scale to develop better services for our patient population of approximately 72,000 patients.

Whitby Health Partnership is a core member Practice of the PCN and the second largest Practice.

The successful candidate will be employed by and based at Whitby Health Partnership focusing on their patient population. They will join a valued and existing team of 3 Advanced Nurse Practitioner's, 13 GP's and 2 nurses here at Whitby Health Partners.

Main duties of the job

The post holder is an experienced nurse who, acting withintheir professional boundaries, will provide care for the presenting patientfrom initial history taking, clinical assessment, diagnosis, treatment andevaluation of care. They will demonstrate safe, clinical decision-making andexpert care, including assessment and diagnostic skills, for patients withinthe general practice. The post holder will demonstrate critical thinking in theclinical decision-making process. They will work collaboratively with thegeneral practice team to meet the needs of patients, supporting the delivery ofpolicy and procedures, and providing nurse leadership as required. In order towork at this level NMC requirements for advanced practice must be met.

Job responsibilities

Main duties of the job

  • Diagnosing and treating patients presenting to primary care with acute medical conditions, minor illness and mental health problems, including a mix of face to face and telephone consultations.
  • Ensuring clinical practice is safe and effective and working within boundaries of competence.
  • Initiating and arranging appropriate investigations and referrals to other members of the primary and secondary health care teams as clinically indicated.
  • Participating in the administrative and professional responsibilities of the practice team.
  • Ensuring the clinical record is completed and maintained with accurate and contemporaneous details recorded.
Person Specification
Qualifications
  • Fully registered with the appropriate clinical body and have experience of working within primary care.
  • Prescribing qualification and be able to demonstrate continued professional development and professionalism.
Experience
  • Excellent communication and interpersonal skills are a must, as well as the ability to work effectively in a team.
  • Ability to demonstrate innovation and awareness of future opportunities and challenges would be desirable to help meet the current and future challenges of primary care, and to help us promote a rewarding environment for staff as well as ensuring excellent patient care.
