A leading healthcare organization in Rochdale is seeking a Clinical Pharmacist to enhance patient care through expertise in medication management. The role involves conducting clinical reviews, managing prescriptions, and collaborating with healthcare teams. Ideal candidates will have a pharmacy degree, clinical diploma, and independent prescribing qualification, along with a commitment to health equity. This position offers full-time and part-time options, contributing to a multidisciplinary team focused on quality improvement.
The post holder, as an employee of Rochdale Health Alliance, will work within the Rochdale North Primary Care Network Clinical Pharmacist service for GP practices as part of a multidisciplinary team.

Providing expertise in clinical medicines management through face-to-face structured medication reviews, management of long-term conditions, transfer of care, and systems for safer prescribing. Manage repeat prescription authorizations and reauthorizations.
The post holder will provide leadership on quality improvement and clinical audit around medications, contributing to the Quality and Outcomes Framework and local contract performance indicators.
The postholder will undertake the CPPE pathway (or have already completed this).
