Full Time Pharmacist

Miles Platting, Newton Heath & Moston PCN is seeking to recruit a pharmacist to complement our current clinical team. This role will be based within GP practices in the Primary Care Network.

Main duties of the job

Main Duty of the Job

  • Assist patients with medication-related problems and queries
  • Support general practice staff with repeat prescriptions and medication queries
  • Collaborate with the prescribing team on safety and quality of practice prescribing
  • Conduct medicines reconciliation, including requests for new medications (e.g., discharge summaries/outpatient letters) and review of repeat medications for newly registered patients
  • Support completion of necessary training such as independent prescribing if not already qualified
  • Develop skills in areas of interest (e.g., respiratory, diabetes, minor illness)
  • Perform structured medication reviews for all patient cohorts
  • Undertake clinical audits to support patient care and safety
  • Participate in contract-related work to help the practice achieve targets for QOF, IIF, and prescribing incentives
  • Assist with chronic disease diagnosis, management, and reviews (e.g., hypertension, diabetes, CKD, hypercholesterolaemia), depending on experience and training

Job responsibilities

The pharmacist will work as part of the multidisciplinary team in:

Person Specification
Qualifications
  • Bachelor's degree in pharmacy and registration with a professional body
