Job summary

Miles Platting, Newton Heath & Moston PCN is looking to recruit a pharmacist to join our current clinical team. These roles will be based within GP practices in the Primary Care Network.

Main duties of the job

Main Duty of the Job

The pharmacist will work as part of the multidisciplinary team to:

1. Assist patients with medication-related problems and queries
2. Support general practice staff with repeat prescriptions and medication queries
3. Collaborate with the prescribing team on safety and quality of practice prescribing
4. Perform medicines reconciliation, including requests for new medication (e.g., discharge summaries/outpatient letters) and reviewing repeat medications for newly registered patients
5. Support with completing necessary training such as independent prescribing, if not already qualified
6. Develop skills in areas of interest such as respiratory, diabetes, minor illness
7. Conduct structured medication reviews for all patient cohorts
8. Undertake clinical audits to support patient care and safety
9. Participate in contract-related work to help the practice achieve targets for QOF, IIF, and prescribing incentives scheme
10. Assist with chronic disease diagnosis, management, and reviews (e.g., hypertension, diabetes, CKD, hypercholesterolaemia), depending on experience and training
