Job description

The Patient Safety Investigations Manager and Patient Safety Specialist is responsible for leading patient safety incident investigations and ensuring that the organization learns from these incidents to improve overall patient safety.

The post holder will work closely with various stakeholders, including clinical and governance teams, to identify learning and improvement opportunities and enhance the patient safety culture.

Main Duties of the Post Holder Include

1. Conducting and overseeing complex patient safety incident investigations to ensure thorough systems-based analysis, including embedding human factors and just culture principles.
2. Developing and delivering training programmes related to patient safety incident investigations to enhance staff skills and knowledge.
3. Providing senior leadership and support on highly complex issues pertaining to patient safety incidents.
4. Working closely and collaborating with various stakeholders at all levels of the organization to foster a positive patient safety culture.
