The Patient Safety Learning Response Lead at a leading NHS Trust plays a vital role in implementing the Patient Safety Incident Response Framework. This position involves coordinating incident investigations, analyzing safety intelligence, and designing patient involvement initiatives, focused on enhancing patient safety across multiple services.
The Patient Safety Learning Response Lead (Insight and Involvement) plays a central role in the Trust’s implementation of the Patient Safety Incident Response Framework (PSIRF). This role supports various elements of our safety management system, including patient safety oversight, analysis of safety intelligence, and development of Patient Safety Partners across the Trust.
As part of the Learning Response Lead team, the Lead will coordinate all Patient Safety Incident Investigations (PISIIs) in line with PSIRF expectations. The role involves engaging with stakeholders to identify learning opportunities, using tools grounded in human factors, systems engineering, psychology, and investigation best practices. Sensitive and compassionate engagement with patients, families, staff, and others affected by incidents is essential.
The Lead will manage the Trust Safety Oversight System, ensuring effective integration and presentation of intelligence such as horizon scanning and active surveillance data. They will analyze complex data, draw robust conclusions, and share insights with stakeholders at different understanding levels.
Supporting patient safety profiling and development of system reliability measures to report and escalate safety concerns is also part of the role. The Lead will be responsible for high-quality investigations, working autonomously to deliver timely reports aligned with national guidance and best practices.
Additionally, the role involves collaborating with the Patient Involvement Team to design and implement a network of Patient Safety Partner roles, including relevant training and support, and linking with peer networks and staff Safety Specialists at MFT.
