A leading NHS Trust is seeking a Patient Safety Learning Response Lead to enhance its safety incident management framework. This role involves coordinating patient safety investigations, engaging with stakeholders, and utilizing data insights to drive improvements in patient safety across the Trust, making a vital impact on healthcare quality and patient outcomes.
Job summary
The Patient Safety Learning Response Lead (Insight and Involvement) is a central role in the Trust's implementation of the Patient Safety Incident Response Framework (PSIRF), and supports multiple elements of our safety management system, including supporting patient safety oversight and insight, analysing and integrating multiple sources of patient safety intelligence, and supporting the recruitment, support and development of Patient Safety Partners across the Trust.
The Patient Safety Learning Response Lead (Insight and Involvement), as part of the Learning Response Lead team, will be responsible for leading and coordinating all Patient Safety Incident Investigations (PISIIs) across the Trust in line with the expectations of PSIRF, engaging with a range of stakeholders to identify opportunities for learning using recognised tools encompassing principles of human factors and ergonomics, systems engineering, psychology, and investigation best practice as required. A high level of sensitive and compassionate engagement with patients, families, staff, and others affected by incidents is expected of this role.
Main duties of the jobResponsible for management of the Trust Safety Oversight System, ensuring intelligence including horizon scanning, active surveillance and other data is integrated and presented effectively. Analysing data from multiple sources, understanding complex interactions and ensuring robust conclusions are drawn and shared with a range of stakeholders with different levels of understanding
Responsible for supporting areas of patient safety profiling, where risk has been identified or in support of Trust-wide priorities.
Using in depth knowledge of patient safety data analysis, including an understanding of human factors approaches, to support the development of a range of system reliability measures to support effective reporting and escalation of emergent safety concerns.
Use highly specialist knowledge to be professionally responsible and accountable for own caseload of concurrent systems-focussed, high quality, Patient Safety Incident Investigations (PSIIs) in line with national guidance incorporating best practice identified as part of the implementation of PSIRF nationally, working autonomously in leading the delivery of timely and effective investigation reports
Working with the Patient Involvement Team to inform the design and implementation of a network of Patient Safety Partner roles across the Trust, including access to relevant training and support, links into peer networks and the network of staff Patient Safety Specialists at MFT
