A leading healthcare organization in South East Essex is seeking an enthusiastic Band 6 or Band 7 nurse to join their intermediate care team. This role focuses on facilitating patient discharge and preventing avoidable admissions, offering excellent development opportunities. Candidates should have experience in a multidisciplinary environment and a commitment to effective teamwork.
We are looking for an enthusiastic, dynamic nurse or therapist with the necessary skills, experience, and vision to work within our busy intermediate care team. Based in the South East Essex area, this team facilitates hospital discharge and prevents avoidable admissions by working with our Virtual Frailty Ward and community and acute providers.
We are seeking a Band 6 or Band 7 nurse, depending on experience, to join our team. Candidates should have experience working within a multidisciplinary environment, demonstrate a commitment to effective team working, and preferably have community setting experience.
If you are passionate about intermediate care, interested in patient discharge and admission avoidance, and looking for a challenge, we would love to hear from you. The role involves weekend working. After completing necessary competencies, there is potential to progress to a Band 7 role unless you are applying with the relevant experience to start directly at Band 7.
