Job description

Key Responsibilities

  1. Coordinate PTL meetings for specific specialties and provide cover during periods of leave.
  2. Track designated patients throughout their 18-week pathway, escalating issues of concern promptly.
  3. Assist the project manager in the review process.
  4. Coordinate national and local audits for specific specialties by collating and analyzing data.

General

  1. Enhance capability and capacity for service improvement across the multidisciplinary teams (MDTs).
  2. Facilitate service improvements with MDTs to enhance patient care experience and outcomes.
  3. Collaborate with MDTs to develop their team around the patient journey, ensuring the patient and carer experience is central at every stage.
  4. Operate as part of the operations team to ensure departmental efficiency.
  5. Maintain effective communication, including of sensitive or challenging matters.
  6. Communicate with patients and GPs as required, supporting patient pathways.
  7. Ensure all administrative responsibilities are performed to high standards, monitoring and improving services.
  8. Identify process issues and propose pathway improvements.
