Job summary

St Marys Hospice in Cumbria is seeking a Patient Flow Co Ordinator to coordinate the patient journey from referral through admission, transfer and discharge. You will work with families, doctors and ward teams to ensure clear communication and a smooth care pathway.

The role requires excellent communication and organisational skills, IT literacy with EMIS and Excel, and a compassionate, patient-centred approach. Hours include Mon-Sat shifts.

We’re looking for an organised and compassionate Patient Flow Coordinator to help ensure a smooth and supportive journey for patients and their families.

In this key role, you will coordinate the patient pathway from the point of referral, working closely with families, doctors, ward teams and the discharge coordinators to ensure clear communication and seamless transitions throughout care.

Key Responsibilities
  • Coordinate the patient journey from point of referral
  • Communicate effectively with patients, families and multidisciplinary teams
  • Maintain and track patient information using EMISandExcel
  • Support efficient admissions, transfers and discharges
  • Ensure every patient and family experience is as smooth and supportive as possible

Hours

What we do

St. Marys Hospice is a charity, providing specialist palliative, supportive and care at the end of life for people living with advancing illness, and their families. We have two main bases: the hospice building in Ulverston and the St Marys Living Well Centre in Barrow in Furness. We also provide care in the home setting and provide therapy groups within the local community.

Hospice services include a 6-8 bedded In-Patient Unit, Hospice at Home Service, Family & Bereavement Support Service and Living Well Therapies.

We are also a regional educational hub for palliative and end of life care education and part of the NW Coast Learning Collaborative. We deliver a range of education to internal staff and to the wider health and social care workforce.
